Thank you for entrusting us with your care. As part of your care, you and/or your authorized representative have the right access to your health information. We have made access to your electronic health information available to you through Connected Care Center.
What is Connected Care Center?
Connected Care Center is the program we use to provide you access to your electronic health information.
How do I get access to Connected Care Center?
Let us know if you would like access to your health information electronically (contact information provided below).
We will ask you to sign a Request to Access form which will permitted us to provide you and/or your authorized representative access to your health information.
You will be asked to provide your email address and cell phone number. You will receive an email from Connected Care Center with instructions for accessing your health information.
Is Connected Care Center secure?
We take great care to ensure your health information is kept private and secure.
Your access to Connected Care Center is controlled through the use of a secure activation code sent to you and personal usernames and passwords that you manage.
Is there a cost?
No. Connected Care Center is available at no cost.
